James N. Topper, M.D., Ph.D., Chairman
Dr. James Topper became executive chair of board of directors for Calistoga Pharmaceuticals in [2006]. Dr. Topper joined Frazier Healthcare Ventures in 2003 as a venture partner and became a general partner in 2005. Frazier Healthcare Ventures is one of the nation’s leading providers of venture and growth equity capital to emerging healthcare companies, having invested in more than 100 companies to date. In addition to service on Calistoga Pharmaceuticals’ board of directors, Dr. Topper also serves on the boards of Amicus Therapeutics, Arête Therapeutics, Intradigm Corporation, La Jolla Pharmaceuticals (LJPC), MacuSight Inc., and Zelos Therapeutics, Inc. Prior to joining Frazier Healthcare, Dr. Topper served as head of cardiovascular research and development at Millennium Pharmaceuticals and ran Millennium San Francisco, formerly COR Therapeutics. Prior to the merger of COR and Millennium, Dr. Topper served as the Vice President of Biology at COR and was responsible for managing its research activities. Dr. Topper has also served on the faculties of Stanford Medical School and Harvard Medical School.

Dr. Topper received his medical degree and doctorate in Biophysics from Stanford University. He is board certified in Internal Medicine and Cardiovascular Disease and has authored more than 50 publications.

W. Michael Gallatin, Ph.D., President
Michael Gallatin has been President and a member of the Board of Directors since he co-founded Calistoga Pharmaceuticals in March 2006. Dr. Gallatin has more than 25 years of research and development experience in oncology, immunology, and vascular biology. Dr. Gallatin was one of the founding scientists at ICOS Corporation in 1990, most recently serving as Vice President and Scientific Director of ICOS Corporation before it was acquired by Eli Lilly and Company. Prior to joining ICOS, Dr. Gallatin was a faculty member of the Fred Hutchinson Cancer Research Center in Seattle, and an affiliate faculty member of the Department of Microbiology and Immunology at the University of Washington. He has been a member of the Scientific Advisory Board of the Keystone Symposia and Caprion and currently serves in that capacity for Stromedix Corporation and the University of Texas Department of Chemistry. He is a member of the BioSeek and Seredigm Boards of Directors. Dr. Gallatin also is a Venture Partner to the Biopharma team of Frazier Healthcare Ventures.

Dr. Gallatin received his Bachelor of Science in Zoology from Truman State University and was awarded a doctorate in Immunology from the University of Alberta, Department of Immunology. He undertook his postdoctoral training at Stanford University in the laboratory of Dr. Irving Weissman.

Ed Hurwitz
Ed Hurwitz joined Alta in 2002 and is a director in Alta BioPharma III and Alta Partners VIII. Alta invests in life sciences companies and has funded more than 101 companies in the sector to date. In addition to participating on the board of Calistoga Pharmaceuticals, which he joined in 2006, he is on the boards of FoldRx, MacroGenics, and Cara Therapeutics. He also led Alta’s investment in ChemoCentryx, Threshold Pharmaceuticals, and Sunesis. Prior to joining Alta, he served as senior vice president and Chief Financial Officer of Affymetrix from 1997 to 2002. From 1992 to 1997, Hurwitz was a biotechnology research analyst for Robertson Stephens & Company and Smith Barney Shearson. Prior to that, he practiced commercial law.

Mr. Hurwitz received his Bachelor of Arts in Molecular Biology from Cornell University. He earned his law and Master of Business Administration degrees from the University of California Berkeley.

Richard Lin, M.D.
Richard Lin joined the board of Calistoga Pharmaceuticals in 2006. Dr. Lin is a partner at Three Arch Partners, which has helped create, build, and fund more than 100 healthcare companies. Previously Dr. Lin was a research scientist at the University of California San Francisco School of Medicine and Harvard Medical School. Dr. Lin’s other current board memberships include Spiration, Visiogen, AccentCare, Vein Rx, Sunshine Heart, Seattle Medical Technologies, Gentiae, and LipoScience.

Dr. Lin received training in general surgery at the University of California, San Francisco. He has a bachelor's degree in biology from Stanford University and a doctorate from Harvard Medical School. Dr. Lin also holds a M.B.A. from the Stanford Graduate School of Business.

R. Lee Douglas
Mr. Douglas joined the Board of Calistoga Pharmaceuticals in 2008. He was a co-founder of COR Therapeutics, Inc., which was acquired by Millennium Pharmaceuticals in 2002. While at COR, Mr. Douglas served in a number of key management positions, including as its original CEO, CFO and Vice President, Corporate Development In addition, he co-headed COR's manufacturing project team for its platelet aggregation inhibitor INTEGRILIN, which is approved in major countries worldwide.

Since 1998, Mr. Douglas has been an independent consultant to a number of biopharmaceutical companies. He currently serves on the board of directors of Affymax, Inc. (NASDAQ:AFFY). He was a member of the founding teams of both Portola Pharmaceuticals, Inc, where he is an advisor, and Five Prime Therapeutics, Inc., where he serves on its board of directors. Since 2002, he has also been a Visiting Scholar in the Department of Molecular and Cell Biology at the University of California, Berkeley, where he conducts research on kinesins (cytoskeletal motor proteins) as potential drug targets for the neglected parasitic diseases African sleeping sickness and leishmaniasis.

Before starting COR Therapeutics, Mr. Douglas was a general partner in the venture capital group at Robertson, Colman & Stephens (RCS). Previously he worked in investment banking at both RCS, and Donaldson, Lufkin & Jenrette in New York. He received his M.B.A. from Harvard Business School, MCRP from the Harvard Graduate School of Design and B.A. from University of North Carolina at Charlotte.
